Facility Access Waiver

All students and members who are new to Ohio State and Rec Sports facilities must complete the Facility Access Waiver before their first visit. Facility access cannot be provided until the waiver has been signed.

How to complete the waiver

1. Visit the Online Member Portal.
2. Sign in using your Ohio State name.# and password.
3. Select Facility Access Waiver.
4. Review and electronically sign the waiver before visiting a Rec Sports facility.

Please complete the waiver before arriving to help expedite your access into the facility.

If you need help accessing or completing the waiver, do not have access to a device or need an accessible version of the waiver, visit the RPAC Welcome Center. Staff are available to assist.
