Facility Access Waiver

All students and members who are new to Ohio State and Rec Sports facilities must complete the Facility Access Waiver before their first visit. Facility access cannot be provided until the waiver has been signed.

How to complete the waiver

1. Visit Rec Register.
2. Sign in using your Ohio State name.# and password.
3. Select the clipboard icon labeled Facility Access Waiver.
4. Review and electronically sign the waiver before visiting a Rec Sports facility.

Please complete the waiver before arriving to help expedite your access into the facility.

Affiliate members who need help accessing Rec Register can contact recsports@osu.edu. Most members will sign in using their Ohio State name.#.
